RN Care Coordinator (Case Manager)

Job Summary

The RN Care Coordinator (RNCC) is responsible for managing a caseload of patients, ensuring care that aligns with desired clinical and financial outcomes. The RNCC possesses specialized skills and knowledge pertinent to the unique needs of their assigned patients. They coordinate the care prescribed by an interdisciplinary team, utilizing patient assessments, care guidelines, protocols, payer regulations, and responses to therapies to evaluate the episode of illness from pre-admission through to post-discharge.

Qualifications
