RN Care Coordinator (Case Manager)

The RN Care Coordinator (RNCC) is responsible for managing a caseload of patients, ensuring that care is delivered in a manner that supports optimal clinical and financial outcomes. This role requires specialized skills and knowledge tailored to the unique needs of the assigned patients. The RNCC coordinates care as prescribed by an interdisciplinary team, using a comprehensive approach to patient assessment and care management.

Qualifications

• Current and valid RN license.
