Licensed Practical Nurse (LPN)

Position Overview

We are seeking a dedicated and qualified Licensed Practical Nurse (LPN) to join our primary care team. This position is designed for an individual who is passionate about providing quality healthcare and support to patients in a primary care setting.

Work Schedule

• Day Shift: Monday through Friday
• Hours: 7:00 AM – 4:00 PM
• Weekly Commitment: 40 hours
• Contract Length: 8 weeks

Qualifications

• Must have at least 1 year of relevant experience
• Current New York State license as a Licensed Practical Nurse (LPN)
• Basic Life Support (BLS) certification required

Key Responsibilities

As a Licensed Practical Nurse, you will be responsible for:

• Providing direct patient care and support, including assessments and administering treatments
• Collaborating with healthcare team members to develop and implement patient care plans
• Educating patients and their families on health maintenance and disease prevention
• Maintaining accurate patient records and documentation
• Assisting with various clinical and administrative duties as needed
