Job Description:

As an RN Case Manager in Hospice, you will engage in fulfilling and impactful work that makes a difference in the lives of patients and their families every day. Your role includes developing and implementing care plans, collaborating with the interdisciplinary team, and providing compassionate care to patients with terminal or chronic illnesses.

Responsibilities:

• Develop care plans in collaboration with the patient’s physician, family, or primary caregivers upon admission.
• Periodically review and recertify patient care plans as necessary.
• Communicate and coordinate care plans through interdisciplinary discussions and case conferences.
• Conduct ongoing assessments of patients’ clinical, functional, and emotional statuses to identify problems and monitor progress.
• Document evaluations, patient responses, and progress towards goals clearly and accurately.
• Educate patients and caregivers on care techniques and end-of-life symptom management when appropriate.

Minimum Qualifications:

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license.
• Graduate of an accredited school of nursing.
• Preferred experience in Medical/Surgical/Oncology nursing and/or hospice care.
