Job Summary

The RN Care Coordinator (RNCC) is responsible for managing a caseload of patients and ensuring that the care provided supports both clinical and financial outcomes. The RNCC possesses the specialized skills and knowledge required to meet the unique needs of assigned patients. This role involves coordinating care as prescribed by an interdisciplinary team and assessing the episode of illness from pre-admission through post-discharge.

Key Responsibilities

• Manage a caseload of patients and facilitate the coordination of care across various phases of treatment.
• Utilize patient assessment, care guidelines, and protocols to evaluate patient progress and response to therapies.

Qualifications

• Current RN license required.
