Home Health Registered Nurse (RN)

Job Summary

As a Home Health Registered Nurse, you will be responsible for assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. You will deliver patient care in alignment with the organizational philosophy, policies, and community health practice standards. Additionally, you will execute admission processes and case management responsibilities in accordance with established policies and standards of practice, ensuring timely and thorough electronic documentation and interdisciplinary communication.

Key Responsibilities

• Assess patient conditions and develop appropriate care plans.
• Provide ongoing education and support to patients and their families.
• Perform admission processes and manage cases as required.
• Document all interactions and patient information electronically and accurately.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Travel to assigned cases as needed.
• Participate in weekend rotations every six weeks.

Qualifications

• Minimum of 1 year of certified home care experience.
• At least 1 year of case management experience.
• Current nursing license and certification required.
• Ability to travel to various locations as assigned.

Additional Information

The start date for the position will be determined based on credentialing clearance. If credentialing is not completed by the posted start date, a new start date will be scheduled.
