Physical Therapist

Job Summary: We are seeking a dedicated Physical Therapist to provide exceptional physical therapy services to patients in their residences. This role involves assisting individuals with physical impairments, functional limitations, or disabilities resulting from various causes including injury or disease. The patient demographic ranges in age from birth to over 100 years.

Key Responsibilities:

• Deliver physical therapy services to patients in their home settings.
• Collaborate with patients, families, physicians, and other allied health professionals to assess patient needs and create effective treatment plans.
• Coordinate services to ensure that patient requirements are met in a timely manner.
• Utilize thorough and timely electronic documentation to maintain accurate health records.
• Engage in interdisciplinary communication in accordance with agency policies and procedures.

Qualifications:

• Must hold a valid Physical Therapist Professional license in Massachusetts.
• Minimum of 1 year of travel experience and 1 year of home care experience required.
• Ability to travel to various patient locations as assigned.
