Long-Term Care Registered Nurse (LTC RN)

Job Description

We are seeking a dedicated Long-Term Care Registered Nurse (LTC RN) to join our team. The successful candidate will provide nursing care to residents in a nursing home environment, focusing on the elderly and rehabilitation patients.

Shift Details

• Evening shift from 2:30 PM to 11:00 PM
• Every other weekend and holidays are required

Unit Information

• Unit Type: Long-Term Care/Skilled Nursing Facility (LTC/SNF)
• Number of Beds/Residents: 70, majority private rooms
• Electronic Medical Record System: ECS
• Nurse-to-Patient Ratios: RN 1:20, LPN 1:20, CNA 1:8
• Common Diagnoses: Dementia, frail elderly patients, rehabilitation needs

Key Responsibilities

• Conduct assessments and charting for patient care
• Administer medications and perform clinical skills including IV insertions, catheterizations, and ostomy care
• Fulfill charge duties as required
• Collaborate with interdisciplinary teams including therapy, administration, social workers, activities, dietary, and housekeeping

Required Qualifications

• Must have a minimum of 1 year of experience in a long-term care or similar setting
• Current Basic Life Support (BLS) certification from AHA
• Demonstrated experience working with elderly patients
• Ability to perform IV therapies and manage titratable drips

Work Environment

• Supportive team environment with resources available after hours including physician availability
• Respiratory Therapy on-site 24/7
• On-site pharmacy with a medication dispensing system

Additional Information

• Block scheduling with shifts of 8 hours
• Floating to other units may be required
• Clean and professional dress code; solid color scrubs permitted
• Orientation specific to the unit and department will vary based on skills and experience
