Physical Therapist (PT) – Home Health

Job Description

We are seeking a dedicated and skilled Physical Therapist to join our home health team. The ideal candidate will have at least one year of experience in home health and proficiency in OASIS documentation.

Key Responsibilities

• Conduct assessments to evaluate patient needs and develop personalized care plans.
• Implement treatment programs to assist patients in achieving rehabilitation goals.
• Monitor patient progress and adjust treatment plans as necessary.
• Provide education to patients and caregivers on exercises and rehabilitation techniques.
• Document patient interactions and treatment outcomes in the electronic charting system.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Qualifications

• Minimum of 1 year of experience in home health settings.
• Proficient in OASIS documentation.
• Current Virginia Physical Therapy license required.
• Basic Life Support (BLS) certification required; Manual Therapy certification preferred.

Skills

• Assessment and documentation skills.
• Knowledge of various therapeutic techniques, including AROM, AAROM, and PROM.
• Experience with orthopedic conditions and post-operative care.
• Ability to manage time effectively and prioritize patient care.
• Ability to identify environmental barriers impacting patient rehabilitation.

Patient Population

• Adults and geriatrics with various diagnoses including orthopedic, neurological, and cardiopulmonary conditions.
• Experience with conditions such as Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), and post-stroke rehabilitation.

Work Environment

• Primarily home health visits with a focus on patient-centered care.
• Opportunities for interdisciplinary collaboration in various settings.
• Flexible scheduling available to accommodate patient needs.

Expectations

• Expected to conduct 5 patient visits per day, with a focus on thorough evaluations and effective treatment implementations.
• Participate in weekly team meetings and contribute to continuous improvement efforts.

Additional Information

• Business casual attire or solid color scrubs permitted.
• Weekends are not required but may be utilized to make up for missed weekday visits.
