Speech-Language Pathologist (SLP) Home Health

Job Description

We are seeking an experienced Speech-Language Pathologist to join our home health care team. This position involves working with patients in their homes to evaluate and treat communication and swallowing disorders. The ideal candidate will possess strong skills in OASIS documentation and have a passion for helping individuals regain their quality of life.

Key Responsibilities

• Conduct evaluations and assessments to determine patient needs and develop individualized treatment plans.
• Administer therapy to individuals with speech, language, and swallowing disorders.
• Create and maintain accurate patient records adhering to OASIS guidelines.
• Establish and enforce appropriate therapy schedules to meet productivity goals.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.

Requirements

• Minimum of 1 year of experience in a home health setting.
• Proficiency in OASIS documentation required.
• Current Basic Life Support (BLS) certification preferred.
• Ability to see an average of 6 points per day or 30 points per week.
• Local candidates are welcome and will be compensated at the same rate.

Additional Information

Mileage will be reimbursed at the IRS standard rate.
