Home Health Registered Nurse (RN)

Responsibilities

• Assess patient conditions and implement personalized care plans.
• Provide ongoing education and support to patients and their families.
• Perform admission processes according to established policies and standards.
• Conduct case management responsibilities in alignment with organizational standards of practice.
• Utilize thorough and timely electronic documentation.
• Engage in interdisciplinary communications to facilitate patient-centered care.

Qualifications

• Minimum of 1 year of certified home care experience.
• Minimum of 1 year of case management experience.
• Ability to travel to various cases as assigned.
• Weekend rotations required approximately once every 6 weeks.

Skills

Strong clinical assessment capabilities, effective communication skills, and the ability to collaborate in a team-oriented environment are essential.
