Job Overview

We are seeking a dedicated Licensed Clinical Social Worker (LCSW) for a 13-week assignment. This position will require the ability to float between multiple units, providing essential behavioral health assessments and care management discharge planning support. The ideal candidate will have experience in emergency department behavioral health assessments and care management on medical-surgical floors.

Key Responsibilities

• Conduct behavioral health assessments in emergency department settings for all age groups.
• Assist with care management and discharge planning for patients on medical-surgical units.
• Float between various units, including PWF, PMH, and PNB, as needed.
• Work with interdisciplinary teams to ensure comprehensive patient care.
• Participate in alternating weekend shifts as required.

Qualifications

• Licensed Clinical Social Worker (LCSW) certification is required.
• Experience in conducting acute behavioral health assessments.
• Prior experience in care management is preferred.
• Ability to work flexibly across multiple units and shift rotations.
• Proficient in utilizing Epic for charting purposes.

Work Schedule

This position involves a rotating schedule with both day and night shifts available, adhering to a 4×10 hour format. Weekend shifts will be required on an alternating basis.
