Physical Therapist Home Health

We are seeking a dedicated Physical Therapist with home health experience to join our team. The ideal candidate will possess strong skills in assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.

Key Responsibilities

• Evaluate and assess patient conditions to develop effective treatment plans.
• Implement patient care plans in compliance with organizational policies and community health standards.
• Provide ongoing education and support to patients and their families regarding their rehabilitation and care process.
• Perform admission processes and case management responsibilities in accordance with established practices.
• Ensure thorough and timely electronic documentation and interdisciplinary communication.
• Travel to assigned cases as needed.
• Participate in weekend rotations every six weeks.

Qualifications

• Minimum of 1 year of certified home health experience.
• At least 1 year of case management experience specifically within a certified home health agency.
• Experience in hospice or private care is not considered sufficient for the qualifications.
• Strong communication and interpersonal skills.
• Ability to work independently as well as collaboratively in a team environment.

Start date will be based on credentialing clearance. Please note that if clearance is not obtained by the specified start date, a rescheduled start date will be necessary.
