Physical Therapist (PT)

Job Description

An exciting opportunity is available for a Physical Therapist to join a team dedicated to providing excellence in rehabilitation services. This position involves a 13-week traveling contract primarily focusing on Skilled Nursing Facility (SNF) and Long-Term Care (LTC), alongside some outpatient (OP), home health (HH), and assisted living (ALF/ILF) responsibilities.

Responsibilities

• Provide high-quality physical therapy services to patients in various settings.
• Develop and implement individualized treatment plans.
• Monitor patient progress and adjust treatments as necessary.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Educate patients and families on treatment plans and rehabilitation techniques.

Qualifications

• Current license to practice as a Physical Therapist.
• Strong communication and interpersonal skills.
• Ability to work effectively within a team.

• Flexibility to cover various settings and patient needs as required.

Work Schedule

This position requires availability for Monday to Friday shifts from 8:00 AM to 4:30 PM, with a 30-minute unpaid break. Holiday coverage may be required.
