Physical Therapist

Job Summary

We are seeking a dedicated Physical Therapist to provide therapy services to patients in their residences who are facing physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. The patient population may range in age from birth to over 100 years, requiring a compassionate and adaptable approach.

Key Responsibilities

• Deliver physical therapy services in accordance with the individual needs of patients.
• Collaborate effectively with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate necessary services.
• Maintain thorough and timely electronic documentation, adhering to agency policies, procedures, and standards of practice.
• Ensure seamless communication across interdisciplinary teams to support optimal patient care.

Qualifications

• Must possess a valid Physical Therapist Professional license in Massachusetts.
• Minimum of 1 year of experience in home care.
• Availability for weekend rotations every six weeks.
• Ability to travel to assigned cases as needed.

Unit Description

The role serves multiple locations within the South Region, including towns such as Allston, Belmont, Brighton, Brookline, Chestnut Hill, Newton, Watertown, and many others in Massachusetts.
