Case Management (RN)

Job Summary

The role of a Case Manager (RN) involves coordinating patient care, including discharge planning, to ensure smooth transitions from acute care settings. This position requires an understanding of case management processes and collaboration with interdisciplinary teams to provide optimal patient outcomes.

Job Responsibilities

• Manage and facilitate discharge planning for patients in an acute care setting.
• Collaborate with healthcare professionals to develop and implement effective care plans.
• Assess patient needs and ensure appropriate resources are available for a seamless transition.
• Maintain thorough documentation of patient progress and case management activities.
• Educate patients and families regarding discharge processes and available community resources.
• Adhere to regulatory requirements and organizational policies within the case management framework.

Qualifications

• Currently holding a Massachusetts conditional nursing license.
• In the process of obtaining or applying for a permanent nursing license upon job offer.
• Minimum of 2 years of experience in case management.
• At least 6 months of experience in discharge planning within an acute care hospital setting, preferably in a large organization or teaching hospital of 500 beds or more.
• Strong communication and organizational skills.

Work Schedule

This position requires a commitment of 40 hours per week, with scheduling dependent on operational needs. Potential shifts may include four 10-hour shifts or five 8-hour shifts, Monday through Friday.
