Job Summary

A healthcare organization is seeking a skilled Case Management Registered Nurse (RN) to join their team. This role focuses on discharge planning and case management in an acute care hospital setting.

Shift Details

This position offers a full-time schedule of 40 hours per week, with potential variations based on operational needs, such as 4×10 or 5×8 shifts. The work will primarily be conducted during the day, Monday through Friday.

Responsibilities

• Utilize case management principles to coordinate and manage patient care effectively.
• Develop and implement discharge plans to ensure smooth transitions for patients leaving the acute care setting.
• Collaborate with various healthcare professionals to optimize patient outcomes and facilitate care continuity.
• Document case management activities and maintain accurate patient records in compliance with organizational policies.
• Provide education and support to patients and their families throughout the discharge process.

Qualifications

• Must possess a current MA conditional license or be in the process of obtaining a permanent license.
• A minimum of 2 years of case management experience is required.
• At least 6 months of discharge planning case management experience in an acute care hospital setting, preferably within a large organization or teaching hospital with 500 beds or more.
• Strong communication, organizational, and analytical skills are essential for success in this role.
