Physical Therapist (PT) – Home Health

Job Overview

We are seeking an experienced Physical Therapist (PT) specializing in Home Health to provide high-quality care to patients in their homes. This role requires a dedicated professional with a strong background in patient assessments and therapy interventions.

Responsibilities

• Evaluate and assess patients’ physical therapy needs in their home environment.
• Develop and implement customized treatment plans based on individual patient needs and goals.
• Utilize OASIS data to document patient evaluations, progress, and outcomes effectively.
• Educate patients and families on therapeutic exercises and home care techniques.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Maintain accurate and timely documentation of patient interactions and treatment plans.

Qualifications

• Minimum of 6 months to 1 year of recent home health experience.
• Proven experience with OASIS documentation.
• Current state licensure as a Physical Therapist.
• Strong communication and interpersonal skills.
