Physical Therapist (PT) – Home Health

Job Overview

We are seeking a skilled Physical Therapist (PT) to join our home health team. The ideal candidate will possess a strong background in home health therapy and will be dedicated to providing high-quality care to patients in their own environments.

Key Responsibilities

• Conduct comprehensive patient assessments and evaluations to develop individualized care plans.
• Provide direct physical therapy services in patients’ homes, focusing on maximizing the patient’s mobility and functional abilities.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
• Document patient progress and modify treatment plans as necessary.
• Educate patients and families on treatment plans, exercises, and rehabilitation processes.

Qualifications

• Minimum of 6 months to 1 year of recent home health experience required.
• Must have OASIS documentation experience.
• Current, valid state licensure as a Physical Therapist.
• Ability to travel to patient locations within the designated area.
• Strong communication and interpersonal skills.

Work Schedule

Shift: Day (5×8-Hour, 08:00 AM – 04:30 PM)
