Job Description:

As a Physical Therapist in home health, you will have the unique opportunity to build strong relationships with your patients and become an essential part of their care team. With an average visit duration of 45 minutes, you will have the time necessary to provide impactful therapy. You will enjoy a balance of independence and collaboration within a cohesive patient-care team, allowing you to have a flexible work/life balance.

Responsibilities:

• Evaluate patients’ needs for physical therapy and develop tailored treatment plans.
• Implement appropriate treatment programs in compliance with agency policies and professional standards.
• Communicate effectively with all members of the healthcare team.
• Document evaluations, treatment goals, and objectives in a clear and concise manner.
• Educate patients and their families about the rationale behind treatments and procedures.

Minimum Qualifications:

• Bachelor’s degree in Physical Therapy.
• Licensed Physical Therapist in the states of Vermont and New Hampshire, or possess an unencumbered compact license.
• At least one year of experience as a Physical Therapist in a home healthcare setting.
