Licensed Practical Nurse (LPN)

Job Overview

The Licensed Practical Nurse (LPN) position is focused on providing high-quality home health care services. Working in collaboration with a team of healthcare professionals, this role involves delivering patient-centered care to individuals in their homes.

Schedule

• Monday – Friday: 8 AM – 4:30 PM
• Weekend rotation requirement: 1 out of 4 weekends

Key Responsibilities

• Conduct six patient visits per day to provide skilled nursing care as per established protocols.
• Work in conjunction with registered nurses (RNs), physical therapists (PTs), occupational therapists (OTs), and home health aides (HHAs).
• Perform nursing functions in accordance with the State Nurse Practice Act and applicable standards of practice, as well as federal, state, and local regulations.
• Deliver patient care as delegated by the RN Case Manager and/or Clinical Team Manager.
• Maintain compliance with organizational and departmental policies and procedures.

Qualifications

• Must possess relevant home health experience.
• Current certification in Basic Life Support (BLS).
• Valid licensure as a Licensed Practical Nurse (LPN). Verification of licensure and certification is required.
