Home Health Registered Nurse

We are seeking a dedicated Home Health Registered Nurse (RN) to provide exceptional patient care in a home setting. The ideal candidate will bring a wealth of experience in performing OASIS assessments and care planning for in-home patients.

Responsibilities

• Deliver high-quality nursing care to patients in their homes.
• Conduct OASIS assessments in accordance with established guidelines.
• Develop and implement comprehensive care plans tailored to individual patient needs.
• Coordinate with healthcare professionals to ensure optimal patient outcomes.
• Participate in on-call rotation, providing support at night, weekends, and holidays as required.

Qualifications

• Minimum 2 years of experience in home health care with a focus on OASIS assessments and care planning.
• Previous experience with electronic health record systems (Epic preferred).
• Must be a licensed Registered Nurse in the state of practice.
• Ability to work independently and manage time effectively in a home health environment.

Work Schedule

This position offers day shifts from Monday to Friday, with the requirement to participate in an on-call rotation for nights, weekends, and holidays (one in four).

Additional Information

The position is available for full-time hours ranging from 32 to 40 hours per week.
