Home Health Physical Therapist

We are seeking a qualified Home Health Physical Therapist to provide care and rehabilitation services in a home health setting. The ideal candidate will have a solid background in home care physical therapy with strong knowledge and experience in OASIS and Homecare Homebase.

Key Responsibilities

• Conduct thorough assessments of patients, focusing on geriatric and orthopedic specialties.
• Develop and implement personalized home exercise programs for patients.
• Educate patients and their families on therapeutic techniques and adaptive devices.
• Perform documentation in compliance with OASIS standards.
• Utilize equipment such as gait belts and Hoyer lifts to assist patients effectively.
• Address patient needs related to chronic conditions such as COPD and CHF.
• Collaborate with interdisciplinary teams, including social services and interpretation services.

Qualifications

• Minimum of 2 years of experience in home care physical therapy.
• Current DC Physical Therapy license required.
• BLS certification required.
• Experience with Homecare Homebase and OASIS documentation is essential.
• Must have completed at least 1 travel assignment.
• Ability to float to other zip codes as needed based on patient and staffing requirements.

Work Schedule

This position involves a day shift, working five days a week from 8:00 AM to 5:00 PM.

Additional Information

• Service area covers multiple locations within the region.
• Number of patient visits per day: Approximately 6.
• Mileage reimbursement available for travel between patients.
• Contractors will use their personal vehicles and must ensure they meet all licensure and insurance requirements.
