Registered Nurse (RN)

Job Summary

We are seeking an experienced Registered Nurse (RN) with a strong background in Long-Term Care (LTC) and Patient Care Coordination (PCC). The ideal candidate will be able to start immediately and perform effectively with minimal orientation.

Responsibilities

• Provide high-quality nursing care to patients in a long-term care setting.
• Coordinate patient care activities and collaborate with interdisciplinary teams.
• Monitor patient conditions and implement appropriate interventions.
• Administer medications and treatments as prescribed.
• Educate patients and their families on health management and care plans.
• Maintain accurate and timely documentation of patient information.

Qualifications

• Current and valid Registered Nurse (RN) license.
• Previous experience in Long-Term Care (LTC) and Patient Care Coordination (PCC) is required.
• Strong clinical skills and the ability to work independently.
• Excellent communication and interpersonal skills.
• Ability to adapt and respond to changing situations effectively.
