Job Description

The Licensed Practical Nurse (LPN) position involves providing home health care to patients. The ideal candidate must have prior experience in home health care and the ability to deliver skilled nursing functions in accordance with the State Nurse Practice Act and established organizational policies.

Key Responsibilities

• Perform skilled nursing functions as directed by the Clinical Team Manager.
• Conduct six patient visits per day within a team setting that includes Registered Nurses (RNs), Physical Therapists (PTs), Occupational Therapists (OTs), and Home Health Aides (HHAs).
• Deliver patient care as delegated by the RN Case Manager and/or Clinical Team Manager.
• Adhere to federal, state, and local laws, as well as the guidelines of your professional license.
• Participate in a weekend rotation, working one weekend shift per month.

Qualifications

• Experience in home health care is required.
• Active LPN license and Certification verification.

This position offers a structured schedule of Monday through Friday from 8:00 AM to 4:30 PM, with a weekend rotation requirement.
