Licensed Practical Nurse (LPN)

Job Description

The Licensed Practical Nurse (LPN) position is responsible for providing skilled nursing care in accordance with the State Nurse Practice Act and established standards of practice, while adhering to federal, state, and local laws. Candidates must have prior experience in home health care.

Schedule

This position offers a Monday to Friday schedule from 8:00 AM to 4:30 PM, with a weekend rotation requirement of one out of every four weekends.

Responsibilities

• Deliver nursing care to patients as directed by the RN Case Manager and/or Clinical Team Manager.
• Conduct an average of six patient visits per day.
• Collaborate with team members including RNs, Physical Therapists (PT), Occupational Therapists (OT), and Home Health Aides (HHA).
• Follow established organizational and departmental policies and procedures.
• Perform skilled nursing functions in accordance with the State Nurse Practice Act and professional license guidelines.

Qualifications

• Experience in home health required.
• Certification in Basic Life Support (BLS).
• Current licensure as a Practical Nurse.
