Licensed Clinical Social Worker (LCSW)

Position Overview

This position is for a Licensed Clinical Social Worker, focusing on case management within the Emergency Department. The role involves working with a variety of patient populations, specializing in the Transition of Care Department, which includes disposition planning and addressing Social Determinants of Health (SDOH), domestic violence, substance abuse, and homelessness. The primary focus will be on patients requiring skilled nursing facility (SNF) placement and home health/DME arrangements, particularly for adult neurology patients, including those experiencing strokes and seizures.

Schedule

• Full-time, 5 days a week (Monday – Friday)
• Shift: 8:00 AM – 4:30 PM
• Holidays as needed
• Floating to other departments may be required based on patient needs

Qualifications

• Minimum of 1 year of experience in a clinical social work role is required
• Travel experience is required
• Previous experience as a LCSW in an acute care setting is strongly preferred
• BLS certification is required
• Current North Carolina license is required; LCSW or CSW accepted, and LCSW-A may be considered
• Experience with EPIC software is required
• Experience in case management work setting is required, specifically within the Emergency Department

Key Responsibilities

• Manage patient disposition planning and coordinate care for various patient populations
• Address Social Determinants of Health (SDOH) impacting patient outcomes
• Assist in navigating resources related to domestic violence, substance abuse, and homelessness
• Specialize in arrangements for SNF placements and home health/DME for neurology patients
• Collaborate with interdisciplinary teams to ensure effective patient care transitions
