Registered Nurse (RN)

Job Summary

As a Home Health Registered Nurse (RN), you will be responsible for assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care will be delivered in alignment with organizational philosophy, policies, and standards of community health practice. This role also involves performing admission processes and managing cases in compliance with organizational policies and standards, ensuring thorough and timely electronic documentation and interdisciplinary communication.

Key Responsibilities

• Assess patient conditions and develop individualized care plans.
• Provide ongoing education and support to patients and families.
• Conduct admission processes and manage case responsibilities.
• Document all patient interactions and care provided in electronic records.
• Ensure compliance with established policies and standards of practice.
• Collaborate with interdisciplinary teams to meet patient needs.

Qualifications

• Minimum of 1 year of Certified Home Care experience.
• At least 1 year of Case Management experience.
• Experience must be specifically within a certified home health agency.
• Ability to travel to cases as assigned.

Additional Information

Weekend rotations are required once every 6 weeks.
