Physical Therapist

Job Summary:

We are seeking a dedicated Physical Therapist to provide therapy services to patients in their homes. The ideal candidate will have experience working with individuals who have physical impairments, functional limitations, or disabilities due to injury, disease, or other conditions. Patients may range in age from birth to over 100 years, necessitating a compassionate and adaptable approach.

Key Responsibilities:

• Provide comprehensive physical therapy services to patients in their residence.
• Assess patient needs and develop individualized treatment plans.
• Collaborate with patients, families, physicians, and allied health professionals to coordinate care.
• Document treatment plans and progress clearly and timely following agency policies and standards.
• Utilize electronic documentation systems effectively to ensure accurate record-keeping.
• Participate in interdisciplinary communications to optimize patient outcomes.

Qualifications:

• Massachusetts Physical Therapist Professional License required.
• Minimum of 1 year of experience in home care.
• 1 year of case management experience is required.
• Please note: Experience in hospice, private care, or case management roles outside of a certified home health agency does not fulfill the requirement for home health care management experience.
• Ability to travel to assigned cases as needed.
• Weekend rotations are required once every six weeks.

Additional Information:

The start date is contingent upon credentialing clearance. If credentialing is not completed by the posted start date, a rescheduled start date will be necessary.
