Physical Therapist

Job Description

We are seeking a dedicated Physical Therapist to provide home health services in a full-time capacity during daytime hours. In this role, you will have the opportunity to build strong relationships with your patients and play a crucial role in their rehabilitation journey.

Key Responsibilities

• Evaluate patients’ needs for physical therapy and develop personalized treatment plans.
• Implement treatment programs in accordance with established guidelines and standards of care.
• Collaborate and communicate effectively with all members of the healthcare team.
• Document evaluations, treatment goals, and objectives accurately.
• Educate patients and their families about the rationale behind treatments and procedures.

Minimum Qualifications

• Bachelor’s degree in Physical Therapy.
• Valid Physical Therapist license in the state of practice, or hold an unencumbered compact license.
• At least one year of experience as a Physical Therapist in a home healthcare setting.
• Basic Life Support (BLS) or Basic Cardiac Life Support (BCLS) certification.
• Rehabilitation certifications such as ASHA, CHT, or ACP Training.

Additional Information

This position does not require holiday coverage. Candidates must be willing to provide services across multiple locations as needed.
