Physical Therapist Assistant (PTA) – Home Health

Job Description

Our organization is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our home health team. This position provides an opportunity to work within a rural, community-based program serving patients throughout Virginia’s Eastern Shore. The PTA will deliver high-quality therapy services to adult and geriatric patients in their homes.

Responsibilities

• Collaborate with physical therapists and the interdisciplinary care team to implement tailored therapy plans.
• Provide skilled patient care, assisting patients in their recovery from illness, injury, surgery, and chronic disease.
• Focus on promoting patient independence, preventing hospitalizations, and enhancing overall quality of life.
• Manage and conduct approximately six patient visits per day within a geographically diverse service area.
• Document patient progress and treatment outcomes accurately and timely.

Qualifications

• Current license or certification as a Physical Therapist Assistant.
• Experience in home health care is preferred, particularly working with elderly patients.
• Strong communication and interpersonal skills to build rapport with patients and their families.
• Ability to work independently and manage a flexible schedule.
• Willingness to travel to patients’ homes across the designated service area.

Work Schedule

This position requires availability for day shifts, working five days a week, with eight-hour shifts from 8:00 AM to 4:30 PM.

Additional Information

Mileage will be reimbursed at the IRS rate for travel between patient locations.
