Job Summary
The Occupational Therapist is responsible for evaluating and providing rehabilitation interventions in home settings, supervising patient care, and educating patients and their families.
Essential Duties and Responsibilities
- Evaluate and establish a plan of care tailored to meet patients’ needs in their home environment, coordinating care with other disciplines and obtaining orders for additional services as necessary.
- Provide direct patient care based on the established care plan and physician orders.
- Apply knowledge of evidence-based practice guidelines, Medicare regulations, and Conditions of Participation in assessments and treatment techniques, modifying care as appropriate.
- Master scoring of CMS required OASIS and collaborate with the Quality team for necessary adjustments.
- Maintain a basic understanding of insurance requirements related to Home Health.
- Participate in multidisciplinary conferences to establish relevant care plans, ensuring effective communication with physicians and team members.
- Document all skilled services, adverse events, and relevant communications in the medical record accurately.
- Engage in Quality Assurance Performance Improvement initiatives to help the Agency meet its goals.
- Meet established productivity standards in alignment with agency benchmarks.
- Maintain accurate time logs and mileage records, adhering to agency policies for commuting and logging.
- Coordinate with the scheduler to optimize continuity of care and efficient travel routes.
- Oversee services provided by the Occupational Therapy Assistant and conduct supervisory visits within the required timeframe.
- Participate in care conferences and appropriately plan admissions, OASIS time points, and discharges throughout the patient’s episode of care.
- Maintain licensure requirements, including continuing education standards.
- Practice in accordance with ethical standards defined by the occupational therapy licensure board and agency policy.
- Participate in agency-provided educational opportunities.
- Perform additional duties as assigned by supervisor.
Minimum Qualifications
- Bachelor’s degree or higher from an accredited Occupational Therapy program.
- Current license to practice in the State of Montana.
- Valid driver’s license and appropriate auto insurance.
- A minimum of two years of previous Occupational Therapy experience is preferred.
