Shift Information

Typically Monday through Friday, 8:00 AM to 5:00 PM, with occasional evening and weekend coverage.

Job Summary

The Physical Therapist is responsible for evaluating and providing rehabilitation interventions in the home setting. This role includes managing and supervising patient care as well as educating patients and their families.

Essential Duties and Responsibilities

  • Evaluate and establish a plan of care to meet patients’ needs in the home environment, coordinating care with other disciplines and seeking orders for additional services as necessary.
  • Provide direct patient care based on the established care plan and physician orders.
  • Apply knowledge of physical therapy evidence-based practice guidelines, Medicare regulations, and Conditions of Participation during evaluations, re-evaluations, and treatment techniques, modifying treatment as needed.
  • Master scoring of the CMS-required OASIS and collaborate with the Quality team to implement necessary adjustments.
  • Acquire and maintain a basic understanding of insurance requirements in Home Health.
  • Perform permitted interventions under the physical therapy practice act, which may include wound care.
  • Participate in multidisciplinary conferences to develop relevant care plans for patients and communicate appropriately with physicians and other staff members.
  • Document all skilled services provided to patients, adverse events, and relevant communications in the medical record using point-of-care documentation.
  • Engage in Quality Assurance Performance Improvement initiatives to assist the agency in achieving its goals.
  • Manage and meet productivity standards in line with agency expectations.
  • Maintain accurate time logs and mileage, following agency policies for commuting and logging of mileage.
  • Communicate with the scheduler daily regarding work activities.
  • Maximize continuity of care and optimize travel routes when preparing schedules.
  • Oversee services provided by Physical Therapist Assistants and conduct supervisory visits within required time frames. Supervise CNAs when RN involvement in the care plan is no longer necessary.
  • Participate in relevant care conferences and strategically plan admissions, OASIS time points, and discharges throughout the patient’s episode of care.
  • Maintain compliance with physical therapy licensure requirements, including continuing education standards.
  • Practice within the expected standards of ethical behavior as defined by the physical therapy licensure board and agency policies.
  • Participate in agency-provided educational opportunities as scheduled.
  • Perform other duties as assigned by the supervisor.

Minimum Qualifications

  • BS degree or higher from an accredited Physical Therapy program.
  • Current license to practice in the State of Montana.
  • Valid driver’s license, reliable transportation, and appropriate auto insurance.
  • Minimum of 1 year of previous experience as a physical therapist is preferred.
