Physical Therapist (PT) – Home Health

Job Overview

We are seeking a dedicated and compassionate Physical Therapist to provide home health services to patients in the region. This position primarily serves adults and geriatric patients, focusing on rehabilitation following joint replacements and managing chronic health conditions.

Responsibilities

• Conduct home health visits and provide physical therapy services to patients.
• Collaborate with healthcare teams to develop and implement individualized treatment plans.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and their families on treatment plans and rehabilitation exercises.
• Maintain accurate and timely documentation of patient care and progress.

Experience and Requirements

• Minimum of 1 year of experience in physical therapy; first-time travelers are welcome.
• Required certifications: BLS and a valid state PT license (Compact PT is acceptable).
• Experience in home health therapy preferred, particularly with joint replacements, COPD, CHF, and T2DM.

Compensation and Benefits

• Mileage reimbursement at the IRS standard rate, tracked through Home Care Home Base (HCHB) or Epic.
• Flexibility with scrub color; any color is acceptable.

Work Schedule

• Full-time position with a typical schedule of 5 days per week, 8 hours each day.
• Availability to work major holidays as required.
• Maximum of 7 days of requested time off (RTO).
• Floating within facility/scope of practice may be necessary.

Compliance and Orientation

Full compliance is required to start on time. If not compliant, orientation may be pushed back by two weeks due to the bi-weekly orientation schedule. Candidates may need to complete an online HCHB course prior to starting if they lack experience, with associated costs paid upfront.

Patient Census and Productivity

The average daily census is around 175 patients, with a goal of 6 visits per day and full-time productivity around 25 units.
