Home Health/Hospice Registered Nurse (RN)

Job Description

A Home Health/Hospice Registered Nurse (RN) provides skilled nursing care to patients in their homes. The nurse will focus on delivering compassionate and effective healthcare services to individuals with various health conditions, including but not limited to Congestive Heart Failure (CHF), Diabetes, Renal Disease, and Wound Care.

Shift Details

• Shift Type: Days
• Shift Length: 8 hours
• Work Schedule: Monday to Friday, 0800-1630
• Rotating Weekends and Holidays: Required
• On-Call Duties: One weekend on-call per month and one evening on-call per week

Patient Care Responsibilities

• Provide skilled nursing care to home health and hospice patients.
• Manage a typical caseload of approximately 6 patients per day, including admissions and follow-up visits.
• Conduct assessments, wound care, and IV therapy management.
• Perform OASIS documentation proficiently.
• Collaborate with healthcare teams to ensure high standards of care.

Qualifications

• Minimum of 1 year of recent Home Health experience.
• Proficiency in OASIS documentation.
• Experience in wound care and infection control practices in the home environment.
• BLS certification required.
• Ability to travel to patient homes in assigned areas.

Support and Resources

Registered Nurses will have access to both management support and administrative assistance after hours as needed. Certified Nursing Assistants (CNA) are also available to support patient care.

Additional Information

• Shift scheduling is managed by the office, expecting a five-day work week unless designated for a rotating weekend.
• May involve floating between home health and hospice settings.
• Attire: Solid color, clean, and professional attire is required.
