Home Health Registered Nurse (RN)

Job Summary

As a Home Health Registered Nurse, you will be responsible for assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered in accordance with organizational philosophy and standards of community health practice. You will also perform admission processes and case management responsibilities in alignment with home care policies, utilizing thorough and timely electronic documentation as well as interdisciplinary communications.

Key Responsibilities

• Assess patient conditions and implement appropriate care plans.
• Provide ongoing education and support to patients and families.
• Perform admission processes and case management duties.
• Utilize electronic documentation per organizational standards.
• Communicate effectively with interdisciplinary team members.

Qualifications

• Minimum of 1 year of Certified Home Care experience.
• Minimum of 1 year of Case Management experience.
• Experience in hospice, private care, or as a Case Manager not qualifying for the Certified Home Health Care Management experience will not be considered.
• Ability to travel to assigned cases as necessary.
• Weekend rotations required once every 6 weeks.
