Home Health Registered Nurse (RN)

Job Summary

As a Home Health Registered Nurse (RN), you will be responsible for assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. You will deliver patient care within the organization’s philosophy, policies, and standards of community health practice. This role involves performing admission processes and case management responsibilities in accordance with organizational policies and standards, utilizing thorough and timely electronic documentation and interdisciplinary communication.

Responsibilities

• Assess patient conditions and develop and implement individualized care plans.
• Provide ongoing education and support to patients and their families.
• Perform admission processes as per organizational standards.
• Manage cases in accordance with established policies and standards of practice.
• Ensure thorough and timely electronic documentation of patient care activities.
• Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
• Travel to assigned cases as needed.

Qualifications

• 1 year of certified Home Care experience.
• 1 year of case management experience.
• Ability to travel to various cases as assigned.
• Availability for weekend rotations every six weeks.

Skill Requirements

• Current registered nurse (RN) license.
