Case Management Registered Nurse (RN)

Job Overview

We are currently seeking a dedicated Case Management Registered Nurse (RN) to join our team. The ideal candidate will have a strong background in case management, particularly in discharge planning within an acute care setting. This position is full-time, with scheduling dependent on operational needs, potentially requiring 4×10 or 5×8 hour shifts, Monday through Friday during the day.

Responsibilities

• Conduct comprehensive assessments for patients requiring discharge planning.
• Collaborate with interdisciplinary teams to develop and implement effective discharge plans.
• Ensure all necessary resources and support systems are in place for patient transitions.
• Monitor patient progress and make adjustments to discharge plans as necessary.
• Document all findings, assessments, and interventions in compliance with institutional policies.
• Educate patients and their families about post-discharge care and available community resources.

Qualifications

• Active or conditional Massachusetts Registered Nurse (RN) license.
• Minimum of 2 years of case management experience.
• At least 6 months of experience in discharge planning within an acute care hospital setting.
• Experience in a large organization or teaching hospital with 500 beds or more is highly preferred.
• Strong communication and organizational skills.
• Ability to work effectively in a fast-paced environment and manage multiple priorities.
