Licensed Practical Nurse (LPN)

Job Description

The Licensed Practical Nurse (LPN) provides nursing care services in alignment with their responsibilities, ensuring the well-being of residents in a post-acute long-term care setting.

Principal Duties and Responsibilities

• Observe and report symptoms and conditions of residents.
• Administer medications as prescribed by the physician.
• Provide treatments as ordered by medical staff.
• Count narcotics and maintain accurate records.
• Maintain records reflecting resident conditions, medications, treatments, and progress.
• Transcribe physician orders both written and via telephone.
• Place drug orders as necessary.
• Assist in completing nursing histories, assessments, MDS, and care plans.
• Assist in the orientation and supervision of nursing staff.
• Deliver direct resident care as required.
• Comply with established nursing practices and facility policies and procedures.
• Participate in ongoing in-service education.
• Assume charge nurse responsibilities when needed.
• Prepare and coordinate assignments for Certified Nursing Assistants (CNAs).
• Contact physicians to arrange tests, laboratories, x-rays, and medical appointments.
• Assist physicians during rounds and prepare charts for signatures.
• Check meal trays for accuracy and appropriateness.
• Directly supervise CNAs and ensure quality care.
• Perform all necessary treatments and assessments.
• Handle resident admissions and discharges.
• Notify physicians of any changes in resident status.
• Inform family members and healthcare providers about changes in treatment status.
• Document care and maintain nursing notes accurately.
• Update care plans as required.
• Report to nurse managers.

Physical Requirements

• Able to lift up to fifty pounds.
• Able to push/pull forty pounds.
• Able to stand for up to four hours per day.
• Able to bend, squat, and stoop to floor level.
• Able to kneel and reach heights up to four and one-half feet.
• Able to walk and move intermittently.
• Able to cope with physical and mental job stress.

Required Knowledge, Skills, and Abilities

• Ability to recognize changes in resident conditions and take the necessary actions.
• Proficient in reading, writing, and speaking English in a legible and understandable manner.
• Current CPR/AED certification is required.

Education

Must be licensed as a Registered Nurse or Licensed Practical Nurse in the State of Massachusetts. Graduation from an accredited school of nursing is required.

Shift Information

Day Shift: 7 AM – 3 PM. Holiday coverage may be required.

Experience

A minimum of 3 years of experience is required.

Certifications

BLS/BCLS certification is required.
