Licensed Practical Nurse (LPN)

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing nursing care services in accordance with established practices and facility policies. This role includes observing and reporting changes in resident conditions, administering medications, and maintaining detailed records to ensure quality of care.

Principal Duties and Responsibilities

• Observe and report symptoms and conditions of residents.
• Administer medications as prescribed by a physician.
• Provide treatments as ordered.
• Count narcotics and maintain accurate records.
• Maintain records reflecting resident conditions, medications, treatments, and progress.
• Transcribe physician orders received in writing and verbally.
• Place orders for medications.
• Assist with the completion of nursing histories, assessments, MDS, and care plans.
• Participate in the orientation and supervision of nursing staff.
• Perform direct resident care as needed.
• Conform to recognized nursing practices and facility policies and procedures.
• Participate in ongoing in-service education.
• Assume Charge Nurse responsibilities when necessary.
• Prepare and coordinate CNA assignments.
• Schedule MD appointments and arrange testing such as labs and x-rays.
• Assist physicians during rounds and prepare charts for signatures.
• Check meal trays for accuracy.
• Directly supervise CNAs.
• Conduct all required treatments and assessments.
• Manage resident admissions and discharges.
• Notify MDs and NPs of any changes in resident status.
• Communicate with family members and healthcare providers regarding treatment changes.
• Document nursing notes and care updates.
• Update care plans as required.
• Report to nursing managers.

Physical Requirements

• Able to lift up to fifty pounds.
• Able to push or pull forty pounds.
• Able to stand for up to four hours a day.
• Able to bend, squat, and stoop.
• Able to knee and walk as needed.
• Able to reach heights of up to four and one-half feet.
• Able to see and hear with or without assistance.
• Able to cope with both physical and mental job stress.

Required Knowledge, Skills, and Abilities

• Ability to recognize changes in resident conditions and take appropriate actions.
• Effective communication skills in English, including reading and writing.
• Current CPR/AED Certification.

Education

Must be licensed in the State of Massachusetts as a Licensed Practical Nurse or Registered Nurse. Graduate of an accredited school of nursing is required.

Shift Information

• Evening Shift: 3 PM to 11 PM.
• Holiday coverage is required.

Experience Requirements

A minimum of three years of nursing experience is required.

Certifications / Requirements

Current BLS/BCLS certification is required.
