Physical Therapist (PT) – Home Health

Job Description

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our Home Health team. The ideal candidate will provide high-quality physical therapy services to patients in their homes, focusing on rehabilitation and recovery.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Implement therapeutic interventions to enhance patient mobility, strength, and function.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and their families on home exercise programs and safety practices.
• Collaborate with interdisciplinary team members to coordinate patient care.
• Document patient care activities accurately and timely in compliance with regulatory requirements.

Qualifications

• Current and valid Physical Therapist license in the state of practice.
• Previous experience in home health preferred.
• Familiarity with OASIS assessment requirements is essential.
• Strong interpersonal skills and compassionate demeanor.
• Ability to work independently and manage a caseload effectively.
• Valid driver’s license and reliable transportation.

Additional Information

This position requires travel between patient homes, and mileage will be reimbursed at the IRS rate. The schedule consists of day shifts of 8 hours, five days a week. Local candidates are encouraged to apply.
