Physical Therapist (PT) – Home Health

Job Description

We are seeking a dedicated Physical Therapist to join our Home Health team. The ideal candidate will provide high-quality physical therapy services in patients’ homes, focusing on rehabilitation and enhancing mobility.

Responsibilities

• Conduct comprehensive evaluations of patients’ physical abilities and develop personalized treatment plans.
• Implement therapeutic interventions to improve patients’ mobility, strength, and overall function.
• Monitor and document patients’ progress, making adjustments to treatment plans as needed.
• Educate patients and their families on home exercise programs and mobility techniques.
• Collaborate with interdisciplinary teams to ensure optimal patient care.

Qualifications

• Current Physical Therapist license in the state of practice.
• Experience in home health therapy preferred.
• Knowledge of OASIS documentation requirements.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.

Schedule

This position offers a full-time schedule consisting of five 8-hour shifts from 08:00 AM to 04:30 PM.

Additional Information

Mileage will be reimbursed at the IRS rate.
