Physical Therapist (PT)

Job Description

We are seeking a dedicated and experienced Physical Therapist with expertise in home health care and hospice services. The ideal candidate will have a minimum of 2 years of relevant experience, including proficiency with OASIS assessments and travel for home visits.

Responsibilities

• Conduct patient evaluations and develop individualized care plans.
• Provide outpatient physical therapy services in home settings.
• Manage and adjust schedules to accommodate patient needs and changes.
• Collaborate with healthcare teams to facilitate patient recovery.
• Utilize the HealthWyse electronic medical records (EMR) system for documentation.

Qualifications

• Must possess OASIS experience.
• Home health and hospice experience required.
• Ability to travel between patient locations for home visits.
• Flexible and adaptable to changes in scheduling.
• Strong communication and interpersonal skills.

Schedule

This position is full-time, Monday through Friday, from 8:00 AM to 5:00 PM, with holiday coverage as needed.
