Home Health Registered Nurse (RN)

Job Summary:

The Home Health Registered Nurse (RN) will be responsible for assessing patient conditions, implementing individualized care plans, and providing ongoing education and support to patients and their families. Patient care will be delivered in alignment with organizational philosophy, policies, and community health practice standards. The RN will conduct admission processes and manage cases in accordance with established policies, ensuring thorough and timely electronic documentation and effective interdisciplinary communication.

Key Responsibilities:

• Assess patient conditions and create personalized care plans.
• Implement and monitor care plans to ensure effective patient outcomes.
• Provide education and emotional support to patients and family members.
• Perform thorough admission processes for new patients.
• Manage cases and document all interactions in accordance with organizational standards.
• Collaborate with interdisciplinary teams to coordinate patient care effectively.
• Travel to assigned cases as required.
• Participate in weekend rotations approximately every six weeks.

Qualifications:

• Minimum of 1 year of certified home care experience.
• Minimum of 1 year of case management experience.
• Active registered nurse (RN) license.
• Strong organizational and time management skills.
• Excellent communication and interpersonal skills.

Additional Information:

The start date for the position will be determined based on the clearance of credentialing. If clearance is not obtained by the anticipated start date, a rescheduled date may be necessary.
