Key Responsibilities

• Evaluate patients’ physical condition and develop appropriate treatment plans.
• Administer therapeutic interventions to promote movement, alleviate pain, and enhance functional abilities.
• Monitor patient progress and adjust treatment plans as necessary.
• Collaborate with healthcare professionals to ensure coordinated care and optimal patient outcomes.
• Educate patients and their families on treatment protocols and rehabilitation strategies.
• Maintain accurate documentation and records of patient care and progress.

Qualifications

• Graduate degree from an accredited Physical Therapy Program (approved by the APTA).
• Minimum of one year of physical therapy experience in home health or facility settings preferred.
• Current and unrestricted Physical Therapy Licensure.
• Current CPR certification.
