Licensed Practical Nurse (LPN)

We are seeking a skilled Licensed Practical Nurse (LPN) with previous experience in Long-Term Care (LTC) and Patient Care Coordination (PCC). The ideal candidate should be able to begin work immediately with minimal orientation provided.

Responsibilities:

• Provide direct patient care in compliance with established protocols and standards.
• Coordinate with healthcare team members to ensure optimal patient outcomes.
• Monitor and document patient progress and report changes to the healthcare team.
• Assist with the implementation of care plans tailored to individual patient needs.
• Maintain a clean and safe environment for residents.

Qualifications:

• Current and valid Licensed Practical Nurse (LPN) license.
• Prior experience in Long-Term Care (LTC) and Patient Care Coordination (PCC) is required.
• Strong communication and interpersonal skills.
• Ability to work effectively within a team.
• Willingness to work in a fast-paced environment with minimal orientation.
