Licensed Clinical Social Worker (LCSW)

Position Overview

This position primarily involves work in the Emergency Department with the requirement to float to other locations as needed. The schedule consists of 5×8 hour shifts, Monday through Friday, from 7:00 AM to 3:30 PM, with holiday coverage as necessary.

Responsibilities

• Handle disposition planning and address Social Determinants of Health (SDOH).
• Assist patients dealing with domestic violence, substance abuse, and homelessness.
• Specialize in skilled nursing facility (SNF) placement and coordination of home health and durable medical equipment (DME) arrangements for adult Neurology patients, particularly those recovering from stroke and seizure conditions.
• Provide support to various patient populations as needed, ensuring continuity of care.

Qualifications

• Minimum of 1 year of experience required, specifically in a travel capacity.
• Previous experience as an LCSW in an acute care setting is strongly preferred.
• Experience in case management and emergency department operations is required.
• Current BLS certification is mandatory.
• Active North Carolina LCSW or CSW license required; consideration may be given for LCSW-A candidates.
• Familiarity with EPIC software is necessary.
