RN Care Coordinator (Case Manager)

Job Summary

The RN Care Coordinator (RNCC) is responsible for managing a caseload of patients and ensuring that their care aligns with desired clinical and financial outcomes. The RNCC demonstrates proficiency in understanding the unique needs of assigned patients and effectively coordinates the care prescribed by an interdisciplinary team.

Key Responsibilities

• Utilize patient assessment, care guidelines, protocols, and payer regulations to evaluate the episode of illness from pre-admission through post-discharge.
• Participate in ongoing evaluations of practice patterns and systems to enhance the quality, cost, and satisfaction outcomes of care.
• Mobilize resources effectively to maximize the efficiency of care delivery.

Qualifications

• Registered Nurse (RN) license is required.
• Experience in case management or care coordination is preferred.
• Strong analytical, communication, and organizational skills are essential.
