Licensed Practical Nurse (LPN)

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a position in Long-Term Care (LTC). This role involves providing essential nursing care while ensuring the well-being of residents in a nursing home setting.

Key Responsibilities:

• Manage admissions, discharges, and transfers of patients to and from the facility.
• Provide clerical support as needed, maintaining accurate records and documentation.
• Administer medications and treatments as prescribed by the healthcare team.
• Collaborate with healthcare professionals to develop and implement individualized care plans.
• Monitor and assess patient health, reporting any changes to the care team.
• Support residents and their families, providing education and emotional support.
• Adhere to all regulatory and safety guidelines to ensure a safe environment.

Qualifications:

• Current Licensed Practical Nurse (LPN) license in Wisconsin.
• Basic Life Support (BLS) certification required; AHA only.
• Minimum of one year of experience as an LPN in Long-Term Care (LTC) is required.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
• Flexibility to work day shifts, including every other weekend and some holidays.
