Physical Therapist

We are seeking a dedicated and professional Physical Therapist to provide home care services to patients with physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. The ideal candidate will have a strong commitment to patient care and the ability to work collaboratively within an interdisciplinary team.

Key Responsibilities:

• Deliver physical therapy services to patients in their residences.
• Assess patient needs and develop tailored treatment plans.
• Collaborate with patients, families, physicians, and allied health professionals to coordinate care.
• Implement therapeutic interventions and monitor patient progress.
• Maintain thorough and timely electronic documentation according to agency policies and standards.
• Participate in weekend rotations approximately every six weeks as scheduled.
• Travel to different patient locations as assigned.

Qualifications:

• Current Physical Therapist Professional license in Massachusetts.
• A minimum of one year of home care experience is required.
• Ability to communicate effectively with patients, families, and healthcare professionals.
• Strong organizational skills and attention to detail.
• Ability to work independently and as part of a team.
